Merakey is seeking a **Program Coordinator** working with youth and adolescents in community residential rehabilitation host homes as well as youth and adolecents residing in foster care, serving **North Centeral & North Western Pennsylvania**
The Program Coordinator is responsible for monitoring all activities of the caseload to include intake; service planning; health planning; educational and vocational planning; safety planning; and permanency planning, all within agency and state standards and regulations.
The Program Coordinator is responsible for activities described in the Program Description, including case coordination, skill building interventions with youth, monitoring the child's progress, implementing interventions from the treatment and support plans, and assisting with supporting and training host home / foster families. In collaboration with other team members, the Program Coordinator monitors to assess stressors in the host home / foster home, provide technical assistance to families and insure adherence to treatmetn and support planning.
